Becoming a New Patient
We are excited that you have chosen Chapel Hill Children and Adolescents' Clinic for your child’s medical care. Your child’s well-being is our highest priority. Please remember to arrive at least 15 minutes early to your appointment to complete the forms. You may also fill out the new patient forms and bring them to your appointment. Take a moment to meet our team! Please note, we are unfortunately unable to establish care for new patients who do not show for their first appointment with our office. Although we see existing patients through age 21, we do not accept new patients 17 years old and older.
Before you become a New Patient
Do we accept your child’s insurance? We accept most major insurances such as BCBS, AETNA, CIGNA, TriCare, UHC, MedCost, and Medicaid.
Have you selected one of our providers as the primary care physician (PCP) on your HMO plan?
If needed, please call the number on the back of your insurance card to check your PCP is assigned and in network.
What to bring to your first visit
Current Insurance Card.
Updated immunization record.
Newborn discharge summary and any testing done at the hospital (for newborn visits only).
School or camp forms you need filled out.
Medical records. Please bring medical records for us to review or have them sent to us from your previous pediatric office. This helps us deliver better care to your child and understand what has been done before.
Print and complete our 6 New Patient Forms. If you do not have access to a printer, then you can complete these forms when you arrive at the appointment. You will need to arrive at least 15 minutes early, however.
Important note for parents:
It is extremely important that you add your new baby to your health insurance policy once your baby is born. Most insurance plans have a 30-day grace period for newborns to be added. Once the grace period has expired, if the baby has not been added, the parent will be responsible for charges incurred.
Please remember to bring your insurance card, copay, and completed New Patient Forms to your first visit.

You may have heard news about rising numbers of measles cases in Texas and other areas of the United States. 2 doses of the MMR vaccine are highly effective (97%) at preventing measles infections. At CHCAC, children receive their first dose at age 1 and a second dose at age 4, to make sure kids are fully protected as soon as possible, and following the recommendations from the CDC. Some infants aged 6 months to 11 months who travel internationally to high-prevlance areas may need a dose to protect them, but they still need the 1 year and 4 year old doses as well. If you are ever concerned about a possible exposure to measles, please CALL before entering our office. DO NOT ENTER the office. This is a highly contagious illness and special precautions must be taken to prevent spread.
